At your first participation/registration of Booyah, we ask you to sent along the Forms. These don’t have to be signed when sent. You can place your signature when you are on the event.
You don’t have to provide the forms for each event. Because of privacy laws, we are allowed to preserve the documents for 7 years. It is your own responsibility to inform us of any changes.
